Default Diet & Workout log

Hi. I am looking to make a log I can take everywhere. A trainer I had a million years ago gave me one and it was perfect. It was a 8.5x11 folded in half book, it had the meals you ate that day and the workout you did as well on one page. Does anyone know where I can get a printable diet & workout log?

Why not just take a normal notebook and write all that stuff into it? I just use whatever notebook's lying around for my logs.
