Neck Exercise Device Survey - please help!

As part of a school project, I have created a survey to gauge interest in an innovative and affordable neck exercise device. The device is still under development and I'm not selling anything but your feedback could really help me better define the design, cost, and consumer interest. Just my opinion, I think it would be more of a physical sports type of thing. Wrestling, football, mma, even boxing.
