No need to ever max out in my opinion. Keep a journal and record what weight you use on every lift and how many reps per set. Your goal in the gym should either increase the reps with the same weight, and also to start adding weight. Also, for 12 sets (if I read it right) it should take you no more than an hour. Anything past that, and your body will start eating your muscles for fuel. In other words, it'll be counterproductive.
