I agree with you and I did lift this way until a few years back when my rotator tore and I really wished I'd had a better grip on the bar when it did. But, the guy in this video sets himself up for this desaster by bouncing the bar around in his hands just before the lift. Whether it had anything to do with the spotter or not is unknown because the weight that is on the bar could be something he had done easily for reps prior to this happening and the spotter could have been comfortable letting him go with it on his own.
