Default Workout Worksheet

Does anyone have a good worksheet I could use to track my progress?

Excel, AppleWorks, Lotus, Word, OpenOffice are okay for me.

I just scribble on a little notepad that I carry in my pocket. That's if I feel like it.

I used Word but I rarely remember to add stuff onto it. I just know my goal and everytime I lift think how far away I am.

I got a little note pad to write things down in the gym. I'll work on something for the computer this weekend. Shouldn't be too hard. I was just hoping someone would have something premade.
