Along with Shoulder Savers by Eric Cressey here is another good one from the great folks at DieselCrew.com

Both of these are about Pre-Hab. If you have a present injuries your needs may differ. But if you want to keep your shoulders healthy or you have already re-habbed an injury and want to help prevent a recurrence, this is the info you need.

Jimmy Smith also has a lot of rehab vids on youtube. I wouldn't suggest you just follow the routines blindly without knowing as much as possible about your problem and what you may or may not need but it is a great resourse for viewing the exercises:
